Application Disclosure 

As part of the application process there are a few questions we require you to answer and information you are required to provide. All questions asked and information requested are relevant to the Position(s) you are applying for, as well as our legal and client obligations. Before continuing with your Application, we ask that you read and consent to the following, noting that it is a requirement for all Applicants applying for a Position(s) with our organisation to complete in full and truthfully and correctly, this Application Disclosure.

We have no obligation to consider your Application or contact you further in relation to your Application in the event you submit an incomplete Application Disclosure, or you fail to include or attach the required information. 

Any false or misleading statement or information in connection with your Application for employment is considered serious and may lead to:

  • a rejection or non-progression of your application for employment;
  • withdrawal of any offer of employment made; or
  • disciplinary action, including but not limited to immediate termination of your employment, if your employment has commenced with us.

GBA is committed to providing a safe working environment. As part of this commitment we need to ensure that prospective employees are able to meet the inherent requirements of their Position and can perform the required duties safely.

Failure to disclose any and all relevant physical, medical and/or any other illness, injury or condition, and/or any past or pre-existing illness, injury or condition suffered by you which may affect your ability to fulfil the Position(s) applied for, may result in you being dis-entitled to claim compensation pursuant to the relevant workers compensation laws in respect of any further illness, injury or condition recurrence, exacerbation or aggravation of this injury, illness or condition. In particular, section 571B of the Workers Compensation and Rehabilitation Act 2003 (Qld) (the Act) states that if requested by a prospective employer, a prospective worker (you) must disclose to the prospective employer (us) your pre-existing injury or medical condition, if any. If you, as a prospective worker, knowingly make a false or misleading disclosure, under section 571c of the Act, you (or any other claimant) will not be entitled to compensation, or to seek damages for any event that aggravates the pre-existing injury or medical condition. 

Application Disclosure - Acknowledgement and Agreement

You confirm that you have been provided with a copy of the Position Description for the Position(s) you are applying for, and you understand the functions and duties of that Position(s).

You agree and understand that if your Application for employment with GBA is successful, and you are employed in the Position for which you have applied or in any other position, you:

  1. warrant and declare that the information in this Application Disclosure and in all other documents completed by you in support of your Application for employment with GBA, and the information provided by you during all interviews in connection with your Application for employment, is true and correct in every detail;
  2. understand and agree that any progression of your Application and/or offer of employment to you with GBA will be expressly on the basis that the information you have provided in relation to your Application for employment is true and correct in every detail;
  3. agree that the information provided in this Application Disclosure may be checked by GBA with the relevant authorities, third parties and/or sources;
  4. agree that the personal and sensitive information provided by you will be held for the purposes of administering any employment you attain with GBA. You understand and agree that GBA may be required to disclose such personal and sensitive information it holds regarding you to other third parties for purposes related to any employment. You consent to GBA collecting, storing, using, handling and/or disclosing such personal and sensitive information;
  5. understand that any false or misleading statement or information in your Application or interviews for employment on any matter relevant to your Application including (but not restricted to) your qualifications, experience, ability, physical and/or mental health, professional or personal integrity, may lead to:
  • a rejection or non-progression of your Application for employment;
  • withdrawal of any offer of employment that has been made; or
  • disciplinary action, including but not limited to immediate termination of your employment, if commenced; and/or
  • not being entitled to compensation and/or to seek damages for any event that reoccurs, aggravates, accelerates, exacerbates and/or deteriorates a non-disclosed pre-existing injury, illness, disability, disease and/or medical condition.

You also confirm that you have read and understood the Privacy Terms and Conditions and our Privacy Policy, and consent to your personal information (including your sensitive information) being collected, stored, used, handled and disclosed in accordance with the Privacy Terms and Conditions and our Privacy Policy.
